If you missed Meeting #1, you should stop by the RE Office and meet with Jay Cuasay, Director of Christian Formation, to pick up your materials and your child s assignment. Parents must fill out this form for their child to go on retreat. Retreats are SEP 19th or SEP 26th. The dates are assigned<br>by class/catechist. If parents are<br>still unsure what date is assigned to their child, they can contact Jay Cuasay.</td> <td><img SRC="../images/spacer.gif" height=20 width=15><br><a href="../sacraments/Sponsor_Slip_2009.pdf"><img SRC="../images/document_icon_3.jpg" height=100 width=100></a><br><b>Confirmation Sponsor Slip</b><p>This form asks for the candidate's Confirmation Name and Sponsor Name. It also asks for height/<br>weight for ordering the Confirma-<br>tion Gown. Money for the gown is collected as part of the Retreat Fee. Most have completed this slip, but some have not. Also, some may need to make a change over the summer. This form will help them<br>do that. </td></tr> </table> <p><b>IF YOUR CHILD IS PREPARING FOR FIRST COMMUNION AND CONFIRMATION:</b> <p>Your child will celebrate the two sacraments of Confirmation and First Communion at the same Mass celebration in Fall 2009. You should encourage your child to celebrate the Sacrament of Reconciliation prior to his or her First Communion.
